Students graduating from HEARTS from 1994-2007 may request an official high school transcript by filling out the form below.
There is a $5 charge for each transcript request, payable by check sent to HEARTS, PO Box 42126, Cleveland, OH 44130.
Transcript will be mailed once payment is received.
